Hours of work:        Up to 50 hours per month

Hourly rate:               $53.44

Under the direction of the Director of Dietary Services and Information Technology, this position will be responsible for ensuring that the nutritional needs of our residents are being managed.  This will be provided in a manner that is consistent with GROVE PARK HOME’s Mission, Service Philosophy, department and facility policies and procedures and other applicable regulations and guidelines.  This position will be responsible for providing clinical support to the Dietary Services department.

 

QUALIFICATIONS/ EDUCATION/EXPERIENCE:

 

Any combination of education and experience providing the required skill and knowledge for successful performance would be deemed qualifying.  Mandatory and preferred qualifications are as follows:

Undergraduate degree in foods and nutrition from a recognized university or equivalent.

Dietitians of Canada accredited Dietetic Internship or equivalent approved experience or graduate degree in nutrition.

Registration with the College of Dietitians of Ontario.

Membership with Dietitians of Canada and appropriate networks

Minimum of 1 year experience in long term care facility

Knowledge of Ministry of Long-Term Care nutritional services standards

Demonstrated knowledge of appropriate interventions in nutritional care in a LTC setting, including but not limited to dementia, dysphasia, tube feeding, weight management, wound treatment, diabetes, renal disease, food allergies and intolerances

Ability to conduct in-service education sessions

 

DUTIES AND RESPONSIBILITIES:

 

•           Assess all new residents on admission and establish a plan of care; assess all resident as per their annual RAI assessment schedule

•           Review and approve the menu cycle including therapeutic modifications

•           Assess and follow residents identified at high nutritional risk and provide follow up care.  review nutritional interventions regularly for effectiveness

•           Provide nutrition assessment/consultation for any resident of the Home as required

•           In consultation with the Director of Dietary Services and Information Technology ensure that regular quarterly assessments are completed and  
 documented according to the established schedule

•           Attend Resident Care Conferences, when RD is available

•           Be a member of the Program Advisory Committee and be part of the multidisciplinary team process

•           Review new, revised, and updated policies and procedures related to nutritional care as prepared by/with the Director

•           Provide clinical in-services to staff according to agreed upon identified needs by the Director and the RD

•           Provide support to Dietary Services including menu reviews, special projects

•           Interpret and reinforce the Fixing Long Term Care Homes Act, 2021 (FLTCA), Ontario Regulation 246/22.

•           Be involved in MLTC Proactive Inspections as part of the health care team

•           Auditing nutritional care plans to ensure good compliance

•           Prepare care plans for to reflect expectations outlined in Nutrition and Hydration Inspection Protocols as per Proactive Inspection by MLTC

•           Create improved quality indicators as per the 2019 Dietitians of Canada Best Practices guidance

•           Assist the Dietary Supervisors with completion of moderate nutritional assessments

•           Provide in-services to Nursing and Dietary staff in relation to education pertaining to nutrition and hydration

•           Provide support to Medical Director and Nurse Practitioner with clinical decisions pertaining to nutrition

•           Interpret and reinforce the Fixing Long Term Care Homes Act, 2021 (FLTCA), Ontario Regulation 246/22.

•           Be involved in MLTC Proactive Inspections as part of the health care team

•           Provide residents and their families with health teaching to ensure safe dining experiences

•           Track and report on statistics as per Quality Improvement initiatives; including and not limited to referrals, supplements, weight loss/gain and wound
  management

•           Participate as a member of the Quality Improvement Committee
